New Patient Registration
If you want to join our practice you will be required to complete a New Patient Form and questionnaire which can be available from the reception and you will need to provide a valid proof of address (e.g. utility bill) and a proof of ID (e.g. passport, driving licence).
For patients that are new to the country and are from outside the EU, you will also be required to provide a valid passport and visa.
Once you have complied with these requirements one of our receptionists will make an appointment for you to see ou Practice nurse for a new patient health check. Following your health check your details will be entered onto our practice list and you will receive your new medical card from the registration department at the health authority in about six weeks.
Crosshall catchment area
It covers up to Crittals Corner and down as far as the Priory School within a circular radius of the surgery. The catchment area has recently been extended to Petts Wood (bounded by Hazelmere Road Queensway).
